HACCP Provider Survey

Thank you for your interest in the Hearing Aid Coverage for Children Program (HACCP).

The HACCP was launched on July 1, 2021 and offers benefits to children (ages 0 – 20) with no health insurance or whose existing health insurance does not cover over $1,500 per year in cost for hearing aids and related services.

This survey allows interested fee-for-service Medi-Cal providers to be listed in the HACCP provider directory. To ensure provider listings contain current information, please complete the survey below.
